Reminder: tax form
As required by the Affordable Care Act (ACA), if you are enrolled in a DICK'S Sporting Goods medical plan, you will receive a copy of IRS Form 1095-C in January 2017. It includes information you will need for filing your 2016 income taxes. Please do not discard this important tax form, and do not file your taxes until you have received it. Read more.

SSNs required for all covered dependents
ACA regulations require the Company to report all covered associates and dependents to the IRS. If you don't provide SSNs, you could be subject to a penalty on your federal taxes next year.

Tax Form 1095-C: Save It!
What's Form 1095-C?
Form 1095-C is a tax form you can expect to receive before filing your 2016 taxes. It is a tax form required by the Affordable Care Act (ACA). This form will play an important role in your annual tax filings. Form 1095-C shows the months of the year that you and/or your dependents were offered or enrolled in medical coverage through DICK'S Sporting Goods. (This form will not replace any state forms you may receive providing proof of medical insurance.)
Why do I need a Form 1095-C?
In short, you need it to file your taxes. Proving that you're adequately insured each year is required under the Affordable Care Act (ACA). If you aren't covered, you could face tax penalties—which is why it's important to keep this form with other important tax documents you'll need for the current tax year.
How will I receive my form(s)?
A communication will be distributed in mid-January that will have details on how to register to receive an electronic version of your 1095-C. If you do not register for an electronic version, a paper copy of the 1095-C will be sent to your current address on file. Be sure to update your current address on file in myHR Information.
Part III of my Form 1095-C is blank. Why isn't my medical coverage showing?
The 1095-C form you receive from DICK'S Sporting Goods only shows coverage in a DICK'S Sporting Goods medical plan. If you enrolled in coverage other than DICK'S Sporting Goods medical coverage (e.g., through your spouse's employer), you will receive a separate 1095-C form from your other medical plan.
