November 1 through November 19 is your only opportunity to make elections and changes to your benefits for 2019.

Be sure to print your Enrollment Summary after you make your elections.

New Tobacco Surcharge

Beginning in January 2019, teammates who use tobacco products* and enroll in any of our medical plans will pay a surcharge of $23.08 per pay. The surcharge is in addition to the regular medical plan premium and will appear on your pay stub.

How it works

When you enroll for medical coverage during Annual Enrollment, there will be a question about your tobacco status. You won't have to pay the surcharge if:

You've been tobacco-free within the last 3 months or

You enroll in the ComPsych smoking cessation program between December 1, 2018 and December 19, 2018.

*Tobacco products include:

  • Smoking Tobacco
  • Smokeless Tobacco

For 2019, you'll be verifying only your own tobacco status (not your spouse's or children's). Tobacco status will be self-certified during the enrollment process and if you enroll in a medical plan.

If you identified as a tobacco user and are enrolled in a medical plan, you can avoid the surcharge by enrolling/participating in the tobacco cessation program.

Contact ComPsych to enroll in the tobacco cessation program at 1-800-337-9852. The enrollment period to avoid the surcharge is between December 1, 2018 and December 19, 2018.

Changes to PPO1, PPO2 and PPO3

The cost of medical and prescription drug claims to DICK'S Sporting Goods rose again dramatically in 2018, so we are making some changes to our medical plan offerings for 2019. See the 2019 Benefits Plan Summary for the updates that include slight increases in deductibles, copays, and out-of-pocket maximums. You can help to keep costs low for yourself and for DICK'S Sporting Goods by staying in-network, shopping for the highest quality, lowest cost providers and making your health a priority by getting an annual physical, preventive screenings and living a healthier lifestyle.

New premium rates

The cost of health care continues to rise each year. That means the amount deducted from your pay will increase for 2019. There's no difference in premium cost between the Aetna and the Highmark BCBS network (for example, the premium for PPO1 is the same whether you choose the Aetna or Highmark BCBS network.)

2019 Dental and Vision at 2018 Premium Rates

The rates for dental and vision will not be changing for 2019.

New Options and Lower Rates for Supplemental Life

MetLife is our new partner in 2019 for Life and Disability insurance.

You may now elect 6 or 7 times your annual earnings in Supplemental Life Insurance (the current limit is 5 times annual earnings) to a maximum of $1,000,000.

During Annual Enrollment, no medical questions (evidence of insurability) will be required for new participants electing one times annual earnings and current participants electing up one level of coverage. Evidence of insurability will be required for coverage of more than $400,000.

Higher Flexible Spending Account (FSA) contributions

You can contribute up to $2,650 in the Health Care FSA for 2019. There's no change in contribution limits to the Dependent Day Care FSA. Need more information about how your FSA works? Click here.

To enroll or not to enroll?

Review your benefits and make sure you have the right coverage for you and your family for 2019. And if you want medical coverage next year, you must re-enroll. Take a look:

Medical YES You will have NO coverage in 2019.
Health Care FSA YES You will have NO FSA in 2019.
Dependent Day Care FSA YES You will have NO FSA in 2019.
Dental No Your 2018 election will automatically roll into 2019.
Vision No Your 2018 election will automatically roll into 2019.

Your To Do List:

  • Enroll in Benefits text messaging for reminders on important deadlines
  • Get your Playbook at your location beginning November 1
  • Re-enroll in a medical plan – if you do nothing, your coverage will be waived in 2019
  • Re-enroll in the Flexible Spending Accounts (FSAs) – your current enrollment will not roll over to 2019
  • Have your dependent verification ready for any newly added dependents for 2019
  • Identify as a tobacco or non-tobacco user
  • Submit your elections and print a confirmation statement

What Do I Need to Know?

Check to make sure your prescription is covered in 2019

If you enroll in any of our medical plans, Express Scripts will provide your prescription drug benefits. This means:

You'll get an ID card from Express Scripts that you'll use only for prescriptions.

There will be only one formulary (the list of generic and brand-name drugs covered by your pharmacy benefits).

Express Scripts will make sure you're getting the most effective and safe medications for your condition. They'll contact you or your doctor if they see something that requires attention.

To make sure your local pharmacy is part of the Express Scripts network and to see if your medication is on the formulary, visit the Express Scripts website. Express Scripts offers savings at thousands of participating retail pharmacies throughout the country and through mail order.

Covering your dependents

Covering a domestic partner? You can cover your qualified same- or opposite-sex domestic partner and their children under your medical, dental, vision and dependent life benefits. There are some specific rules about eligibility. Get the details.

Tax Form 1095-C

As required by the Affordable Care Act (ACA), if you are enrolled in a DICK'S Sporting Goods medical plan, you will receive a copy of IRS Form 1095-C. The form will be mailed in January 2019. It includes information you will need for filing your 2018 income taxes.
